This notice explains how Preventative Medicine by Science handles information collected through this website, including the appointment request form and website analytics.

It is separate from our Notice of HIPAA Privacy Practices, which describes how we handle protected health information created or received in the course of providing care. If you are an established patient, that notice governs your health record.

The appointment request form asks for your first and last name, email address, phone number, preferred contact method, and a short description of your reason for visit. We use this only to contact you about scheduling.

Email and web forms are not secure channels for health information. For that reason, the reason-for-visit text you enter is not forwarded to us by email; a member of our team will ask for any clinical details by phone or during your visit. Please do not include diagnoses, medications, or other sensitive health details in the form.

If you consent to analytics, we use Google Analytics 4 to understand how visitors move through the site — for example which pages are viewed and which buttons are used. This measurement is limited to non-identifying interaction data such as page paths, referral sources, approximate location derived from IP address, and device or browser type.

We do not send the contents of form fields, your name, or any health information to analytics. We do not use the site to build advertising profiles, and no advertising pixels are installed.

Analytics cookies are set only after you accept them in the consent banner. If you decline, or ignore the banner, no analytics cookies are written and no measurement data is sent.

You can change your mind at any time by clearing this site's data in your browser settings, which removes the stored choice and prompts the banner again. Browser-level tracking protection and Google's own opt-out tools also work here.

We do not sell or rent your information. We share it only with service providers who help us operate the site and communicate with patients, and only to the extent needed to do that work.

Appointment request details are kept only as long as needed to respond to your request and to meet our record-keeping obligations. We use administrative and technical safeguards, including encryption of the connection to this site, to protect the information we hold. No method of transmission over the internet is completely secure.

This website is intended for adults. We do not knowingly collect information from children through this site. If you believe a child has submitted information to us, contact us and we will remove it.
